Care Coordinators provide direct, client- centered case management services, empowering clients to access housing, resources, and opportunities that promote stability and reintegration.
Responsibilities Client Assessment and Planning:
Conduct comprehensive assessments of clients' needs, strengths, and goals.
Collaborate with clients to develop individualized care plans that address housing, employment, healthcare, and social support needs.
Resource Coordination:
Connect clients with housing resources, mental health services, substance abuse treatment, job placement programs, and other essential resources.
Provide guidance and assistance in navigating complex systems and accessing available benefits and services.
Advocacy and Support:
Advocate on behalf of clients to help them overcome barriers to stability and successful re-entry.
Offer emotional support and crisis intervention as needed.
Assist clients in making informed decisions about their lives and goals.
Goal Monitoring and Follow-Up:
Regularly review and adjust client care plans to ensure they remain relevant and effective.
Track and document client progress toward goals, outcomes, and service utilization.
Conduct follow-up meetings and maintain ongoing communication with clients.
Collaboration and Networking:
Collaborate with community partners, government agencies, and other stakeholders to access additional resources and support for clients.
Attend case conferences, team meetings, and community outreach events as necessary.
Data Management and Reporting:
Maintain accurate and confidential client records and case files.
Generate and submit regular reports on client progress, outcomes, and service utilization.
Ensure compliance with all relevant documentation and reporting requirements.
Crisis Management:
Respond to crises and emergencies involving clients, including issues related to housing, health, or safety.
Implement crisis intervention protocols and coordinate emergency services as needed.
