Quick Apply 2 hours ago Full Job Description The Clinical Screener, Behavioral Health (BH) will contact selected membership and assess/screen them to determine if members present with signs and symptoms of an undiagnosed and untreated Behavioral Health or Substance Use Disorder Condition(s) using tools embedded in qHMO.
The BH screener will screen for unmet BH needs and will refer the member for comprehensive behavioral health care and management.
A.

ESSENTIAL JOB FUNCTIONS:
Work Position:
Sitting - Frequency - 90% or more.
Standing - Frequency - less than 10%.
Walking - Frequency - less than 10%.
Driving - Frequency - minimal Travel - Frequency - minimal Body Movements:
Lifting - 0-20 lbs; books, reams of paper, files, etc.
- Frequency - approximately 20%.
Bending, stooping and squatting - Frequency - approximately 15%.
Reaching above head to 75 degrees - Frequency - approximately 15%.
Hand/grip dexterity - Frequency - approximately 50%.

SPECIFICS:
(100% of time is dedicated to PBH) Complete telephonic behavioral health screenings as appropriate.
Identifies patients meeting Behavioral Health (BH) criteria to be a) placed in the Behavioral Health case management programb) referred for an in home assessment Facilitate Safety and Welfare checks as appropriate.
Complete APS reports as appropriate Complete assessment scheduling as appropriate Provide patient education and training as appropriate Maintain a collaborative relation with the medical group staff to ensure patient education, referrals for appropriate trainings and other avenues/efforts to assist in patient compliance.
Works as liaison between member and assessing providers.
Maintain computerized tracking systems and databases.
Generate various reports to constituents as appropriate.
Compile statistics and generate various reports to constituents as requested Assist BH department supervisor/managers as directed.
Other duties as assigned.
The pay range for this position at commencement of employment is expected to be between $19.
00 and $20.
00/hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements:
3
years of Behavioral Health experience preferred.
Active California LPT or LVN license.
Knowledge of Behavioral Health industry practices with preferred emphasis on managed behavioral care Quick Apply.
Estimated Salary: $20 to $28 per hour based on qualifications.
